Pension administrators (like PTAD or private PFAs) require this affidavit periodically (usually every 6 or 12 months) to ensure that pension payments are not being made to accounts of deceased individuals. It serves as a fraud prevention mechanism. This affidavit is also sometimes required by insurance companies or during legal proceedings where the physical existence of a claimant must be verified by the court.
